Medication Administration by School Personnel
The Little Miami Board of Education recognizes that the dispensing of prescribed medication to students is necessary from time to time. The dispensing of the medication is done in lieu of the parent and may be done by teachers, building administrators, school nurse, school secretary, or authorized school personnel.
The Little Miami Board of Education has determined that prescription and/or
nonprescription drugs will be administered under the following conditions:
l. All prescription and nonprescription drugs must be presented BY THE PARENT or guardian to the proper authority (principal or his designee,
secreary, school nurse or teacher) in the original container.
2. The "Authorization for the Administration of Medication by School Personnel Form" must be completed by the parent and physician and returned with all prescription and nonprescription drugs.
3. The prescription and/or nonprescription drug must be presented in the original container and clearly marked as to drug name, dosage, doctor's name, and student's name.
4. A parent and/or guardian may administer over-the-counter nonprescription drugs to their child or children at their request in a designated area provided by the school administrator.
